Opportunity Scholar Documents

If you have received an award letter stating you are a pending Opportunity Scholar, please download & complete the Contract Release Waiver (pdf).
If you received a letter stating you are a Chase Scholar, please click here for the proper documents.

In addition to the Release Waiver, you will also need:

  • Acceptance letter from the educational institution to which the scholarship will be paid.
  • Financial aid award letter/financial aid package/local scholarships offered in attending college.
Please create ONE PDF document that includes ALL of your necessary info listed above, including the Contrtact Release Waiver, and email it to This email address is being protected from spambots. You need JavaScript enabled to view it. by May 24th, 2019.
Please put your full name in the subject line of the email.

Your single pdf should include:

  • Completed Contract Release Waiver (download)
  • Acceptance Letter
  • Additional Funding Information

Dual credit information is not included yet, once the academic year is concluded we will need your dual credit transcript(s).

If you do not have Adobe Acrobat to create pdf's, there are many free resources to do so online.  Search for "free pdf generator online" or something similar. There are even apps for your handheld devices (phone, tablet, etc.) that can convert any photo or document into a pdf. Search in the app store for "pdf generator" or something similar.

 After your first college semester (fall 2019), the Advisory Committee will review the following to determine your eligibility to receive the Opportunity Scholarship:

  • Semester GPA
  • Core classes taken
  • Minimum 15 credit hours

If awarded, the Chase Scholarship will begin the following semester (spring 2020).

If you become a Chase Scholar in the Spring 2020 semester you will be required to participate in our 2020 Community Service.

On Monday, July 15th & July 22nd, from 9-11am the Chase Foundation staff will host an informational meeting on the Opportunity Scholarship at Central Valley Electric, 1403 N. 13th Street, in the Community Room.
